Registration Forms

• Pre-Registration Form - Fill in by February 15.
• Payment – send one payment form per school with accompanying check by March 15
• Please submit the names of all students that will be coming to the fair by using this form by April 11.
• There will be a $25 late fee charged for schools attempting to register after April 11. No schools will be accepted after April 18.
• The photo consent form and MUST be signed by the child’s parent or guardian if he/she is going to participate in the Fair. This is not optional. It must be filled in and brought to us at the registration on the day of the Fair.
